Position Description: Demonstrates advanced skills in assisting families with access to healthcare services and programs available at SYHC, Inc. Increases the medical and dental visits at all sites with new and already existing patients. Helps families to obtain and retain Health Families & Medi-Cal health plan membership. Has advanced skills in helping families with barriers to obtain their insurance and how to keep it.
Qualifications: High School diploma or GED. Additional courses in health care or social services
preferred. Knowledge of healthcare delivery system. CA driver's license with appropriate auto
insurance. Bilingual (English/Spanish) a must. Excellent verbal and written communication skills.
